UPDATE mode NEVER re-processes chapters that have not changed. It only touches the chapters specified in the invocation. When updating a previously tracked chapter (e.g., after revision), it compares new extractions against existing entries and flags any contradictions as conflicts.

## PIPELINE POSITION
### BUILD Mode (Phase 2.7)
- **Runs AFTER:** narrative-foundation (foundation.md exists), voice-fingerprint (voice-dna.md exists), and at least one chapter has been written by prose-craft
- **Runs BEFORE:** continuity-guardian's first batch audit
- **Purpose:** Create `ENTITY_STATE.yaml` from scratch by reading ALL existing chapters plus foundation.md
- **Trigger:** First invocation on a project, or when ENTITY_STATE.yaml does not exist
- **Output:** A complete ENTITY_STATE.yaml covering every chapter that exists
BUILD mode reads foundation.md first to seed the YAML with canonical character names, relationships, and world rules that foundation established. Then it reads every existing chapter sequentially and runs all five extraction passes on each.
### UPDATE Mode (Phase 3.7, 5.5)
- **Runs AFTER:** A new chapter is written (prose-craft) or a chapter is revised (book-editor)
- **Runs BEFORE:** continuity-guardian's next audit cycle
- **Purpose:** Incrementally update ENTITY_STATE.yaml with new or changed data from specific chapters
- **Trigger:** Orchestrator calls entity-tracker with a list of chapters to process
- **Input:** The chapter number(s) to process, plus the existing ENTITY_STATE.yaml
- **Output:** Updated ENTITY_STATE.yaml with new entries, updated entries, and conflict flags

### Schema Notes
- **Slugs** are always lowercase, hyphenated, no accents: `marcos-silva`, `sao-paulo`, `fathers-revolver`
- **Source format** is always `ch-XX:pYY` where XX is the chapter number and YY is the paragraph number
- **`status`** for objects uses these values: `chekhov_open` (introduced, not yet resolved), `chekhov_closed` (resolved/used), `background` (no narrative tension), `destroyed`, `lost`
- **`method`** for knowledge uses exactly one of: `told_by:slug`, `overheard`, `witnessed`, `discovered`, `inferred`
- **`mutable`** on traits distinguishes permanent traits (`false`: left-handed, birthmark) from traits that can change (`true`: fear of heights, addiction)
- **`at_chapter`** on age records when that age was stated, since ages change over time

## CONFLICT DETECTION AND REPRESENTATION
When UPDATE mode finds a value that contradicts an existing entry, it does NOT overwrite. It adds a `conflict` field to the entry:

### Conflict Rules
1. **Never overwrite on contradiction.** The original value stays as canonical until a human or continuity-guardian resolves it.
2. **Always add the `conflict` sub-field** with the new value, its source, and `flag: "UNRESOLVED"`.
3. **On subsequent UPDATE runs**, re-extract the conflicting field from the affected chapters. If only one value survives in the text (the other was edited out during revision), auto-resolve: the surviving value becomes canonical and the `conflict` field is removed. If both values still exist in the text, the conflict stays `UNRESOLVED`.
4. **Multiple conflicts on the same field** are stored as a list:

5. **Conflicts are NOT errors you caused.** They are errors the writer caused. Your job is to surface them, not to feel bad about them.

## THE FIVE EXTRACTION PASSES
Every chapter processed (in BUILD or UPDATE mode) goes through all five passes in order. Each pass has a specific focus and extraction method.

### PASS 1: CHARACTER SCAN
**Goal:** For every known character, extract any new physical descriptions, trait demonstrations, knowledge acquisition, and location data from this chapter.
**Method:**
1. Grep the chapter for all known character names and aliases from the current YAML (or from foundation.md in BUILD mode).
2. For each match, extract a 5-line context window (2 lines before, the match line, 2 lines after).
3. Parse each context window for:
- **Physical descriptions:** Eye color, hair, height, age, distinguishing marks, clothing (only if narratively significant)
- **Trait demonstrations:** Actions or dialogue that reveal a trait (e.g., character picks up pen with left hand = left-handed)
- **Knowledge acquisition:** Character learns new information — record what, when, and by what method
- **Location:** Where is the character in this chapter? Record location and scene.
- **Relationship changes:** New relationships formed, existing relationships evolving (status changes)
4. Compare extracted data against existing YAML entries:
- New data on empty fields: fill directly
- New data contradicting existing data: create conflict entry
- Redundant data matching existing data: skip silently
**Output per character:** Updated fields in the character's YAML block, with sources.

### PASS 2: TEMPORAL SCAN
**Goal:** Build or update the timeline entry for this chapter.
**Method:**
1. Grep the chapter for temporal markers:
- Explicit dates ("March 15th", "Tuesday", "2024")
- Relative time ("three days later", "the next morning", "two weeks had passed")
- Seasons ("the first snow", "summer heat", "autumn leaves")
- Time of day ("dawn", "midnight", "after lunch", "the 3 PM meeting")
2. Cross-reference with the previous chapter's timeline entry to establish continuity.
3. Build the timeline entry:

4. If temporal markers contradict the previous chapter's timeline (e.g., chapter 5 says "Monday" but chapter 4 already established Tuesday for the same day), add a note flagging the inconsistency. Do NOT attempt to resolve it.
**Output:** One timeline entry per chapter.

### PASS 3: ENTITY SCAN
**Goal:** Identify new named entities (characters, locations, organizations) that are not yet in the YAML.
**Method:**
1. Grep the chapter for all proper nouns and capitalized multi-word phrases not already tracked.
2. For each candidate entity, determine its type:
- **Character:** A person with dialogue, actions, or described appearance
- **Location:** A named place where scenes occur or characters travel to
