A rally, a launch, a meeting, a submission, a negotiation,\na phase of work. It went well, badly, or ambiguously — all three are worth reviewing, and\nthe successful ones are the most commonly skipped.\n\nRun it while memory is fresh. A review a week later loses most of the detail that makes\nit useful.\n\n**Purpose**: Convert a completed action into transferable knowledge. `eval` asks \"are we\nmaking progress toward the goal?\" This asks \"what did that specific thing teach us, and\nwhat do we do differently next time?\"\n\nThe discipline that makes an after-action review work is comparing **expected against\nactual**, and taking the gap seriously in both directions. An outcome better than\nexpected is as informative as a worse one, and is almost never examined.\n\n---\n\n## Voice & Tone\n\nFactual and blameless. The purpose is learning, not accounting. Blame ends disclosure,\nand a review nobody is honest in is worse than no review — it manufactures false\nconfidence.\n\nBlameless does not mean vague. \"Comms went out late\" is a finding; \"there were some\ntiming challenges\" is not. Name what happened precisely, and attribute it to a cause\nrather than a person.\n\nWhere a person genuinely didn't deliver, that belongs in `eval`'s people check, not here.\nThis skill examines the system that let it matter.\n\n---\n\n## Execution Sequence\n\n### 1. Load Context\n\nRead `GOAL.json` — the goal, the `plan` key as it stood, the `people` key, the `riskNotes` array, and\nthe log entries covering the period being reviewed. If a `premortem` was run, pull its\npredicted causes: checking them against what actually happened is one of the most\nvaluable comparisons available.\n\n### 2. Establish What Was Expected\n\nBefore discussing what happened, pin down what was supposed to happen. Reconstruct from\nthe plan and log, not from memory — memory reshapes itself around outcomes.\n\n```\nWHAT WE EXPECTED\n  Intended outcome: [from the plan]\n  Success looked like: [the concrete measure, if one was set]\n  Key assumptions: [what the plan rested on]\n  Predicted risks: [from threat or premortem, if run]\n```\n\nIf no expectation was recorded, say so plainly and note it as a finding in its own right\n— an action taken with no stated expected outcome can't be learned from properly, and\nthat's worth fixing before the next one.\n\n### 3. Establish What Actually Happened\n\nFacts and sequence. Resist interpretation at this stage; it contaminates the next step.\n\n```\nWHAT HAPPENED\n  Outcome: [what actually resulted]\n  Against the measure: [exceeded | met | fell short | not measurable]\n  Timeline: [what happened when — only where the sequence matters]\n  Surprises: [what nobody predicted, in either direction]\n```\n\n### 4. Examine the Gap\n\nFor each meaningful difference between expected and actual — including favourable ones:\n\n```\nGAP: [expected X, got Y]\n  Why: [the actual cause, as far as it can be established]\n  Was it knowable in advance? yes | no\n    If yes: what would have revealed it — and why didn't it?\n  Systemic or one-off? [would it recur under the same conditions?]\n```\n\n\"Was it knowable in advance\" is the question that upgrades a review from a recap into\nsomething that changes future behaviour. A knowable-but-missed cause points at a gap in\nthe process, not just bad luck.\n\nCheck the predicted risks explicitly:\n\n```\nPREDICTED RISKS — what actually happened\n  [risk] — materialised | didn't | materialised differently: [how]\n```\n\nRisks predicted that didn't materialise matter too. Either the mitigation worked (keep\nit) or the risk was overrated (recalibrate — and note it, because systematic\nover-prediction wastes effort and credibility).\n\n### 5. Sustain / Improve\n\nThe core output. Both halves are required.\n\n```\nSUSTAIN — worked, do it again deliberately\n  [what] — [why it worked, so it can be repeated rather than re-lucked into]\n\nIMPROVE — change before next time\n  [what] — [the specific change] — [who owns it]\n```\n\n\"Sustain\" is the half people skip. Something that worked by accident and isn't identified\nwill not reliably happen again. Name the mechanism, not just the outcome.\n\nCap \"improve\" at three to five items. A review that produces fifteen changes produces\nnone.\n\n### 6. Elicit\n\nThe user was there. You are working from the file.\n\n```\nThree questions:\n\n  - What's your read on why it went the way it did?\n  - What surprised you most?\n  - Anything that went wrong that isn't in the plan or the log at all?\n```\n\nThat last question matters most. The failures that never reach the written record are\nusually interpersonal or about the user's own capacity, and they're the ones that repeat.\n\n### 7. Check the Goal Still Holds\n\nA completed action is a natural moment to ask whether the goal itself still makes sense\nin light of what was learned. Ask once, without pushing:\n\n```\nDoes this change anything about what you're actually going for?\n```\n\nIf yes, hand to `strategy` — don't renegotiate the goal from inside a review.\n\n### 8. Update GOAL.json\n\nAppend a `log` entry: what was reviewed, the outcome against expectation, and the lessons —\n`notes` isn't rendered in the visual layer, so list findings freely rather than trimming to\nfit a short list; each entry still has its own 120-char cap, so split a long finding across\nmultiple `notes` entries instead of cramming it into one. Fold \"improve\" items into the\n`nextActions` array of whichever line of operation the reviewed event belongs to, under\n`plan.linesOfOperation` — each as `{ action: \"Review: <finding>\", who, when, status: \"pending\" }`.\n`action` is `mediumLabel` (120-char hard cap) and **the `\"Review: \"` prefix counts against\nthat cap** — budget the finding itself to under ~110 chars, not 120, and write it as a short\nlabel (\"tighten comms timeline\", not a full sentence explaining why). If the finding doesn't\nfit in that budget, put the label in `action` and the fuller explanation in that action's\noptional `detail` (max 280 chars, hover-only) instead of lengthening `action`. If the event\ndoesn't map cleanly to one line (or `plan` has only one), add it there rather than guessing a\nsplit. Preserve the `status` already on any existing action in that array — appending new\nfindings is not a reason to touch ones already marked `done` or `dropped`.
