Keep the investigation narrow until the\nfailure layer is known.\n\n## Triage\n\n1. Capture the exact symptom, command, input, output, timestamp, and environment.\n2. Reproduce with the smallest reliable case.\n3. Identify the failing layer: UI rendering, state management, transport,\n   validation, domain logic, persistence, provider, tool execution, synthesis,\n   or infrastructure.\n4. Compare expected and actual structured data.\n5. Form one hypothesis and test it.\n6. Fix the root cause.\n7. Re-run the original reproduction and adjacent regression checks.\n\n## Stop Rules\n\nStop local patching and re-evaluate when:\n\n- Two fixes fail to resolve the same symptom.\n- A fix for one case breaks another case.\n- The failure appears to move between layers.\n- Passing requires more special cases or prompt examples.\n- Mocks pass but real runtime behavior still fails.\n\n## Evidence to Preserve\n\n- Failing test output.\n- Minimal input and actual output.\n- Relevant logs or traces.\n- Diff between previous and current structured data.\n- The command used to verify the fix.\n\n## Recovery Rules\n\n- Do not delete failing tests to get green output.\n- Do not weaken assertions without explaining why the old expectation was wrong.\n- Do not ignore caught errors unless the behavior is intentional and tested.\n- Do not mask race conditions with fixed sleeps.\n- Do not claim a live integration is fixed when only a mock was tested.\n\n## Final Check\n\nReport what failed, what changed, what evidence proves the fix, and what remains\nunverified.\n","tagline":"Guide systematic root-cause debugging.
